To retrieve a copy of your radiology images, submit this request form and a CD will be sent by UPS to your home address. Please allow at least 48 hours notice for all radiology image requests conducted at Cleveland Clinic’s main campus or family health centers.
Please have the following information ready to make the request:
- Date of your exam
- CCF# (How to find CCF#)
- Pick-up or delivery information
- Image type
